Setting up PCVS as your designated agent for NPDB:
You can appoint PCVS as your designated agent by going to the NPDB website.
You will need the last four digits of PCVS entity number with NPDB which is 0077 and our entity name will be Professional Credential Verification Ser (Only Ser will fit). There will be a place where you can select “Query and report”. Please select both Query and Report. You will be asked what type of file PCVS would the reports returned in. Please choose QRXS file.
There is a $2.00 pass through fee, per provider, for PCVS to obtain the NPDB. This is the same fee you would be charged to obtain on your own. Please contact us after you have completed the designation at pcvsmail@pcvs.net.
